Registration for 2013-2014

We currently have openings in the following classes:

5′s Class (M/T/W/Th 9:30am-12pm)

4′s Class: (T/W/Th 1pm-3:30pm)

 

There is a $65 per child, nonrefundable registration fee.  Register via mail or in person.

Listed below are the forms needed for students registered for 2013-2014.

• Medical Release Form

• Health Care Summary (to be completed by child’s physician)

• Immunization Data (to be completed by child’s physician)

• Parent Handbook (final page needs to be signed and returned)

• Emergency Card
